Deliver Reconnect
The Reconnect Pathway restores connection — to self, whānau, community, and the wider world. It provides the wraparound care that allows rangatahi to stand strong, feel safe, and move forward with confidence and purpose.
This pathway is guided by both Te Whare Tapa Whā and Te Mauri Ora, recognising that true reconnection happens when all aspects of wellbeing are in balance — Taha Tinana (physical), Taha Hinengaro (mental and emotional), Taha Whānau (social and relational), and Taha Wairua (spiritual). The Mauri Ora approach ensures that each young person’s life force and potential are nurtured, restored, and strengthened through culturally grounded care, relationships, and opportunities for growth.
Through consistent mentoring, pastoral triage, restorative relationships, and future planning, TOA helps rangatahi rebuild trust and belonging while equipping them to navigate real-world challenges. The first two modules form a core wraparound layer of daily support, while the following modules are activated case by case, depending on individual needs and readiness.
Each completed module earns a Reconnect Milestone — a badge, pen, or small taonga symbolising courage, healing, and community. Completing the full pathway leads to a Reconnect Pathway Certificate of Wellbeing & Support, recognising strength, balance, and connection as essential steps toward Mauri Ora.

Every rangatahi is paired with a dedicated facilitator for daily check-ins and up to 30 minutes of mentoring per week. This consistency builds trust, confidence, and reflection. Mentoring focuses on emotional wellbeing, life balance, and goal setting, helping each student feel supported and accountable.
Learning Milestones:Participate in daily check-ins and weekly mentoring.
Complete a short reflection or wellbeing log.
Set and track one personal growth goal.

Through Pastoral Care Triage, rangatahi can access in-depth support when issues or risks arise. This process recognises the whole person — connecting them to the right services such as WINZ, counselling, youth justice, or health providers. It’s about removing barriers and restoring balance so that learning and growth can continue.
Learning Milestones:Participate in a triage assessment and co-create a support plan.
Connect with at least one external support service.
Reflect on personal wellbeing progress.

This module strengthens the bridge between rangatahi and their whānau or caregivers. Facilitators support clear communication, understanding, and restorative conversations that heal and rebuild relationships. It’s about belonging, accountability, and celebrating progress together.
Learning Milestones:Participate in at least one guided whānau hui or restorative kōrero.
Identify one positive home-based goal or change.
Reflect on what healthy communication looks like in their whānau.

This module ensures rangatahi have a seamless support network between TOA, their case workers, and external service providers. It strengthens collaboration and aligns everyone behind the same goals, helping rangatahi transition smoothly into training, employment, or independent living.
Learning Milestones:Attend or contribute to at least one joint support meeting.
Add case worker and provider goals to their TOA Plan.
Reflect on how teamwork supports personal growth.

As rangatahi progress through TOA, they are guided to create a Future Pathway Plan — a personalised roadmap that connects their interests, goals, and skills to real-world opportunities. It helps them see how far they’ve come and what steps they can take next — whether that’s study, work, or creative enterprise.
Learning Milestones:Design a written or visual Future Pathway Plan.
Set 2–3 achievable post-programme goals.
Present their plan during a closing mentoring session.

For up to three months after graduation, rangatahi are paired with a peer mentor — someone who’s been through TOA and understands the challenges of transitioning back into the real world. This continued support helps maintain accountability, motivation, and connection.
Learning Milestones:Participate in regular peer check-ins for up to three months.
Reflect monthly on progress using a Peer Pathway log or journal.
Celebrate achievements and identify next steps.

When a rangatahi completes their tailored Reconnect modules, they receive a Reconnect Pathway Certificate of Wellbeing & Support and a small taonga representing reconnection — such as a carved pendant or woven band. This closing moment acknowledges resilience, growth, and the courage to keep moving forward. description
